You documented what?

I came across a blog post by Nancy Diggory-Graves over at Crossbonestyle that listed fourteen things that were actually documented as part of a patient care report.
My favorite from the list:
” Discharge Status: Alive, but with out my permission.”

Obviously, the intent of these items are to get you thinking about documentation. Unfortunately, it is an area where many people do not excel. What are your thoughts about documentation and how you feel it could be taught better?
